Senior Options For Independence (SOFI)

Helping you stay independent is the purpose of our SOFI services. SOFI is here to provide support that older adults might require due to persistent illness, recovering from hospitalization, or simply life getting a bit overwhelming at times. In these cases, SOFI provides needed guidance from care management to referrals for specific services. SOFI is available to Fairport/Perinton residents ages sixty and over, as well as residents under sixty who are disabled.

There are no charges for SOFI services, donations are welcome. If you are referred to a service that involves a fee, you will be notified in advance.

ENGAGE NNORC

In 2006, Fairport Baptist Homes’ Community Ministries was awarded a grant from the New York State Office for the Aging to establish a NNORC (Neighborhood Naturally Occurring Retirement Community) within the Town of Perinton, and launched the “ENGAGE NNORC” program. The NNORC is a defined geographically contiguous area containing no senior-specific housing, in which a minimum of 40% of the households contain at least one resident who is 60 years of age or older. The NNORC concept provides both social services and medical support in this area, while working to build a strong community model in which neighbors take care of each other in order to support successful aging in place. The NNORC service area is bounded by Kreag Road to the west, Ayrault Road to the north, Turk Hill Road to the east, and Pittsford-Palmyra Road to the south.

The “ENGAGE NNORC” Resident Advisory Board meets regularly monthly at the Town of Perinton Community Center; part of their ongoing work is to educate and reach out to the “ENGAGE NNORC” community, gathering neighbors’ input for the program’s successful implementation. The Advisory Board also provides input and feedback on the perceived needs of seniors in the “ENGAGE” area, to help with the ongoing development of the “ENGAGE NNORC” initiative, to monitor results with staff and to plan and implement various community activities to enhance participation in the program and promote its success in the community.

SOFI Transportation

One of the most pressing needs in order for seniors to maintain an independent lifestyle is the availability of transportation. SOFI provides door-to-door and wheelchair-accessible transportation services within the Town of Perinton for residents aged 60 and older, for medical appointments and essential errands via the Elderbus. Essential errands include trips to the grocery store, post office, pharmacy, barbershop, hairdressers, bank, etc. The Elderbus runs Monday through Friday between the hours of 8:30 AM and 3:30 PM. Reservations should be made at least one day in advance.

In addition, door to door transportation provided by SOFI volunteer drivers is available for medical appointments outside of Perinton. Wheelchair accessible transportation is not available from Fairport Baptist Homes volunteers, but if necessary, a Fairport Baptist Homes Senior Options For Independence care manager can make a referral to another appropriate program.) Reservations for volunteer transportation service should be made as soon as the medical appointment is made, but no later than 48 hours in advance.

Independent Living

Live independently, with a little help from your friends. Many seniors find themselves fully capable of living on their own, but the stress and work of home ownership simply becomes difficult to manage. That’s why our continuum of care includes Independent Living communities. Fairport Baptist Homes provides a variety of on- and off-campus environments where seniors can maintain independence in caring, friendly communities. Affordable, fully-maintained accommodations – from apartments to one-story townhomes – are available to help meet your needs. And, of course, all services provided by Senior Options for Independence (SOFI) are available to help you remain independent for as long as possible.

The Woodlands

These fully maintained one- and two-bedroom ranch-style townhomes are an ideal, independent housing option for seniors aged 62 and older. These homes are on the main campus and feature an enclosed back patio and one-car garage.

Rehabilitation

When you or a loved one are ready to leave the hospital but not quite ready to return home, our team is here to help.

Our goal is to make your recovery shorter, avoid all bumps in the road, and get you home, healthy. Of course, our Rehabilitation services are available to current residents of Fairport Baptist Homes should they require this level of care.

You and your family will partner with our skilled, experienced staff to create the ideal plan for your recovery. Your Rehabilitation Team can include doctors; nurse practitioners; nurses; licensed practical nurses; social workers; certified aides; dietary specialists; therapeutic recreation specialists; and physical, occupational and speech therapists. Safety evaluations are provided as you progress to confirm you feel steady on your feet, and ready to manage all day-to-day tasks. Our team is also available for a home safety evaluation to ease your transition back home.

During your stay you’ll recuperate in one of our private rooms with a home-like feel in our Transitional Care Center.

To put yourself on the fast track home, simply let your hospital social worker know you want to have your rehabilitation at Fairport Baptist Homes. They will obtain all the necessary information and send it to us:

  • A current PRI (Patient Review Instrument) and Screen
  • Any Health Care Proxy, Power of Attorney and MOLST forms that the patient may have
  • Copies of all the patient’s insurance cards, Social Security card and photo ID
  • When seeking Long Term Placement, we may ask that you also provide copies of current bank statements and other account statements with the completed application

Assisted Living

Our Assisted Living community provides spacious apartments that create a homelike setting. Our apartments are arranged in neighborhoods that give residents the independence they want with the safety and security they need. No long hallways or hospital-style nursing stations exist. Ten to twelve seniors make up a neighborhood where they can socialize, form new friendships and even gather together for meals like a family.

We provide 24/7 support for seniors who are capable of living in an apartment setting but need assistance with meals, laundry, housekeeping , medication, bathing and personal care needs. Each of our three floors has a community dining room, with a fully equipped kitchen, servicing three meals per day, a parlor and a living room/activity area. Light housekeeping and laundry services is included.
 
Residents may bring furnishings from home, and we’re happy to welcome one pet cat as well. Seniors maintain control over their daily schedules remaining as independent and involved as they choose and residents have access to the Fairport Baptist Homes’ continuum of care, should there be a need for a higher level of care, based on availability and individual needs.
 
We are also a licensed Assisted Living Program (ALP) community, for those who are medically eligible and qualify. Our ALP provides all of the services listed above, plus additional support including personal emergency response services, nursing services, a number of home health services and the case management services of a Registered Professional Nurse.

  • Access to higher level of care available through the Fairport Baptist Homes’ continuum of care based on availability and individual needs
  • Fairport Baptist Homes’ volunteer-based transportation services for appointments available with advance notice

Skilled Nursing Care

When around-the-clock care or medical supervision is needed, our continuum provides skilled nursing care. Private and semi-private rooms have been designed in a unique, homelike fashion to create neighborhoods that shatter the idea of traditional “nursing homes.” We helped pioneer this innovative household model decades ago by removing long hallways and hospital-style nursing stations. This community environment, limited to 10-12 residents per household, ensures our residents as much independence as possible with all the care and supervision they need.

Here’s what our Skilled Nursing services include:

  • Three meals daily in a family dining setting
  • Full housekeeping and laundry service
  • A nurse/care manager on-site to develop your individual care plan and coordinate any additional required or desired services
  • On-site nursing support, with certified nursing assistants to help with routine personal care needs
  • Resident directed care
  • Medication management and administration
  • Social, recreational and spiritual activities
  • Beauty salon and barber shop
  • Weekly religious services (Catholic and Protestant) and visits with on-site chaplain
  • Gift shop
  • Around-the-clock access to medical professionals
  • Physical, Occupational, and Speech therapy if needed, following a hospitalization
  • Hospice suite for supportive care and comfort providing additional space and privacy
