Rehab/Transitional Care

Fairport Baptist Homes offers short-term rehabilitative care our 18-bed, private room Transitional Care Center (TCC) for individuals who are ready to leave the hospital but are not yet ready to return home during their recuperation process. Our caring and competent nurses and therapists focus their attention on helping these residents transition from an acute care hospital setting to home as quickly as is safe and comfortable for them.

Our transitional care services include post-operative recovery, extensive wound care, post-stroke care, respiratory or pulmonary management, elective and emergency orthopedic recovery, physical therapy, occupational therapy, speech therapy, comfort care, motor vehicle accident injuries, and management of complex medical conditions.

We work in partnership with our residents, their families, and community services in a comfortable household environment, where residents can recover in an atmosphere of dignity, respect, and positive reinforcement. Our expectation is that the resident will be an active participant in working to improve his or her maximum functional status.

Our transitional care team includes doctors, nurse practitioners, nurses, licensed practical nurses, social workers, certified aides, dietary specialists, therapeutic recreation specialists, physical, occupational, and speech therapists. Our daily goal is to help you regain your strength and independence so that you can return safely to your own home. We provide safety evaluations as you progress, making sure that you feel steady on your feet, ready to work in your kitchen or to manage your daily tasks. We can also visit your home to conduct a safety evaluation before you return and provide guidelines to assist your transition back to home.

You’ll feel comfortable and cared for, recuperating in your private room with a homelike feel, where you can eat communally in a country kitchen, relax in an inviting family room, and even use a washer and dryer. There are flexible visiting hours, and your family and friends can even bring their pets along to see you.

Whether you spend days or weeks recuperating with us, we’ll send you home having reached your maximum potential and level of independent function.

Payment mechanisms include private pay, Medicaid, Medicare, third-party health insurance, and long-term care insurance.

Physical Therapy and Occupational Therapy

Fairport Baptist Homes offers Physical Therapy (PT) and Occupational Therapy (OT) as part of our rehabilitative services.  These therapists work together with the other members of the interdisciplinary team (Social Work, Physicians and Nurse Practitioners, Nursing) to provide these services to the individuals at FBH as needed.

Physical Therapists diagnose and treat residents with medical problems or other health-related illnesses that interfere with residents’ ability to move and perform functional tasks.  PT’s develop a plan and work to reduce pain, restore function, promote an individual’s ability to move, and/or prevent loss of mobility before it occurs.  Interventions include therapeutic exercises, manual and functional therapy, and working with assistive and adaptive devices.

Occupational Therapists work with residents to help improve their ability to perform tasks in their daily living environment.  They assist residents in becoming as independent as possible with such activities of daily living as dressing, cooking, eating, toileting, and grooming.  OT’s also work on improving an individual’s strength and endurance through therapeutic exercises, fine motor coordination and range of motion exercises.

Residents come from Fairport Baptist Homes’ 18-bed Transitional Care Center prior to their going home from an acute care facility.  Many individuals have had orthopedic surgery such as total knee or hip replacements; others have cardiac issues, pneumonia or a variety of illnesses.  Together they work with OT/PT to get back to as independence as quickly as possible so that they can return to their previous level of function.

The OT/PT staff also work with residents who live here for long term care, to forestall any decline in mobility or other functions.

FBH’s staff is skilled and experienced, with many having five or more years of therapy experience in the nursing home setting.
