Adult Day Health Care
LifeTimes Adult Day Health Care, located in the city of Rochester on Plymouth Avenue adjacent to Plymouth Gardens, is operated by the Fairport Baptist Homes Caring Ministries. It is a day program which offers care to adults ages 18+ with medical needs and helps seniors maintain their independence through suppoprtive health and social services in a protective group setting. Assisted Living
Fairport Baptist Homes’ new assisted living community creates an affordable, accessible, and appropriate level of care for older persons in our area who can no longer live safely alone in their own homes. Our assisted living wing, like the skilled nursing units, embraces FBH’s “Pioneer” culture and model of care, offering the same “household” design, the only assisted living community in Monroe County to do so. Community Care/SOFI
Fairport Baptist Homes offers support for elders living independently in the Fairport/Perinton community through Senior Options for Independence (SOFI), a professionally-staffed program supported by volunteers and sponsored by Fairport Baptist Homes in partnership with the Fairport-Perinton Senior Living Council. Founded in 1997, SOFI provides care management and transportation services that support successful aging in place in the Fairport/Perinton community for all residents aged 60 and older. Independent Housing
The Fairport Baptist Homes owns and/or manages a variety of Senior Housing Communities in Fairport. These senior housing options reflect the Fairport Baptist Homes’ commitment to providing seniors with the most affordable housing and care options. In conjunction with the Perinton Churches Housing, Inc., the Fairport-Perinton Senior Living Council, and Fairport Baptist Homes Community Ministries, several housing options are available to area seniors, providing criteria and specific qualifications are met. Rehab/Transitional Care
Fairport Baptist Homes offers short-term rehabilitative care our 18-bed, private room Transitional Care Center (TCC) for individuals who are ready to leave the hospital but are not yet ready to return home during their recuperation process. Our caring and competent nurses and therapists focus their attention on helping these residents transition from an acute care hospital setting to home as quickly as is safe and comfortable for them.
