Outright cash gifts are a common form of giving.  Your cash gift is tax deductible and becomes available immediatly to support the important work of the Fairport Baptist Homes Caring Ministries Foundation.

You may make a cash gift by writing a check  or by using your MasterCard, Visa or American Express card.   Click the Donate button at the top of this page to make a donation on your credit card.  Or, if you do not have a PayPal account, click here for a downloadable donation form that you can fill out and mail in at your convenience to the address below.

Stock Transfers

It’s simple to give a gift of appreciated securities (stock) to the FBHCM Foundation!  Just follow the steps outlined here.  If you have any questions, call the Foundation at (595) 388-2324.

1.  Complete and mail the Stock Donation Letter of Intention given below.  Send one copy to your broker and one copy to FBH via email (Ellen Bevan), fax (585-377-2620) or mail to:

2.  Transfer the Stock. The simplest way to transfer stock is electronically.

1.  Have your bank or broker notify UBS (the Foundation’s investment advisors), either Vincent Battaglia (585-218-4511) or Steve Shemirani (585-218-4514) of the electronic transfer.

2.  Provide your bank or broker with the necessary information to transfer the securities:

  • DTC # 0221
  • FBHCM Foundation Tax ID #  16-1571207
  • FBHCM Account # RA 11388

3.  You will receive confirmation from the Foundation.  When the Foundation’s brokerage firm receives the transfer request, they will send a stock confirmation form to the Foundation stating the value of the shares of stock delivered, the price at the time of sale, and the final value of the gift for tax purposes.  The Foundation will send you a gift acknowledgement which serves as verification and receipt for the securities transfer for your records.

If it is convenient for you to make a gift in person, please visit Fairport Baptist Homes and ask the receptionist to call Ellen Bevan, Director of Advancement or Laurie Pincus, Developement Associate who will be happy to meet with you.

Matching Gifts

Many employers offer matching gift programs and will match the charitable contributions made by their employees. Inquire with your Human Resources Department as to whether your employer will match a tax-deductible contribution to the Fairport Baptist Homes Foundation.  If your company is eligible, there may be paperwork to fill out on both sides, by you the contributor, by the employer and by the Fairport Baptist Homes Foundation.  Procedures vary by employer, but the Foundation staff will assist in making the process a smooth one.  Inquire today!
